Microneedling for hair loss treatment
Microneedling is a type of cosmetic treatment that addresses various effects of aging. It’s also nicknamed skin needling because of its effects on increasing collagen production in the skin.

How it works

Patients with large breasts usually complaint about backache and difficulties to do daily routine. However BREAST reduction is not one technique fit is all type of surgery and the techniques vary according to the size of the BREAST, location of the Nipple related to the BREAST fold ( sagging degree) and the quality of the skin.
Patients without sagging nipple can get a good result with only peri areolar scar or vertical scar depending on the amount of BREAST to be removed. Patients with sagging nipples have to be lifted and the amount and quality of the extra skin will determine the extent of scars. So here is the point. BREAST reduction surgery is a trade off between sagging big breasts and the extension of the resulting scar to rebuild a new breast. Patient with mild to severe sagging of the Nipple will end up with what we call inverted T scar. Patients with bad quality skin as well will have an inverted T as well due to the inability of the skin to retract and pull around the breast after removing glands. So the trade off is clear and it is mainly the scar that need to be explained about to the patients. But there is one more variable that need to be explained. Any breast after been lifted will end up re locating downward because of the action of the gravity
So it will bottom out in diferente degrees and it depends on the remaining skin quality and the size that the patient chose to keep. A different approach to avoid any degree of bottoming out ( meaning any degree of descent of the breasts after reduction and lifting ) is the one where we remove a larger amount of gland and insert a implant that will stay in place which avoid any degree of bottoming out or downward resetting of the breasts. But in this last case patient would have to cope with changing implants after a while and the risk of having capsular contracture as well.
NOTES ON PRE OP PREPARATIONS
Include breasts ultrasound and mammogram that need to be done to check any suspicious or undetected Susie in the breasts , as well to be used for future reference to the patient breasts control imagining. Usually blood testing and any health related problem has to be controlled properly pior to the surgery.
Smoking has to stop 30- 45 days prior to surgery as the risk of Nipple loss skin and opening of the scars during the recovery time is high. Hypothyroidism has major influence too in the healing and scars can open or enlarge due to this health problem.
POST OP CARE
We ask patients to keep their scars clean and to use a bra made of cotton and without iron. Patients are asked to avoid sleeping on their breasts for 4 months to help the newly rebuilt breasts heal in a proper shape and to not force any internal stitches to loosen. Patient will go back to their routine lives after 3 weeks and to drive after 4 weeks. Scars are cleansed at the clinic three times at the first week , twice at the second week and once for the next 2 weeks.
REDUCTION OF THE BREAST WITHOUT SCARS WITH LIPOSUCTION
This can be done with liposuction alone and there are specific patients that may do this procedure. Patients with juvenile big breasts and small to no sagging can be submitted to this procedure. Patients with saggy breasts and nipple sagging cannot be submitted to liposuction for reduction purpose as the changes on the breasts can worsen the visual aspect of the sagging nipples. But in big breast sizes and small to no sagging liposuction is a very good surgery to remove breast gland without scars. One major complication is that the BREAST after liposuction becomes swollen and takes time to get to the result.
BREAST LIFT WITH THREADS
Another procedure breasts that we may apply to lift breasts are threads. However there are very few indications and usually we use them as an adjunct meaning as a support to the surgery itself in some cases. Threads can be absorbable or permanent. Permanent are usually used to support the lower fold under the BREAST when we do major lift in post massive weight loss and we want to raise the fold of the breast and change the imprinting site of the breasts. Absorbable are used to help support breasts after lifting to help support the breats for a while , during the healing time fo the scars to avoid downward pressure on the lower vertical scar so it may not have enlargement. That is empirical and there are no scientific papers proving this outcome.

However there are differences between these two conditions.
People with large amount of fat don’t have usually sagging skin and , if overall health is good , can be submitted to a full body contouring without cutting skin and remodeling their body in a scarless way.
So using modern devices like power assisted liposuction and Vaser , we can remove large quantity of fat with less bleeding and quicker recovery time. We can manage to do liposuction in the abdomem , back and arms , with fat injected to the buttocks and breasts as well to reshape both VASER is used to facilitate such procedure but attention must be paid to avoid excessive timing with the VASER to avoid loss of viable fat or even burns , which unfortunately are becoming common , due to both the patients excessive demands for thinking the skin as much as possible and the doctors wiliness to follow patients demands.
And today we can apply for the same liposuction an extra time with the new J Plasma , which through cold helium , a more precise tightening is done with the skin and tissues. So even after major liposuction , Daley is necessary to remove skin in today’s technical environment a through liposuction will give patients a very sharp and curved waistline allied with fat injected in the hips , a more curvilinear shape that reacts harmony and proportions as well.
POST MASSIVE WEIGHT LOSS and BODY CONTOURING
Sagging skin is a major hurdle that we today face with patients undergoing gastric sleeve or by pass , with a very steep weight loss leading to a thinning of the skin and dermis , and a less reliable skin to retract , with major skin sagging present after 1-2 years of massive weight loss, with multiple folding skin rolls around their bodies , making very difficult to avoid long and unpleasant scars. However , we have managed to correct patients with such conditions , because we discovered that a very agressive liposuction under the skin, by aggressively removing sagging tissues between muscles and skin , can and actually leads to retraction of the emptied skin around the torso. So areas of the back and waistline , that before were removed surgically with ugly scars , today are treated only by extensive long but effective liposuction only treatment.
Folding rolls skin in the back and waistline can be treated with 80-90% success rates. No need in a vast majority of the cases for a lengthy scars anymore. The liposuction with power assisted device and VASER , as well J Plasma helps to bring this outcome. Patients after massive weight loss experience a very significant shrinking of the buttock areas, mainly at the lower half , with some patients having not only sagging around , but hardened skin attached to the fascia underneath the skin at the bottom of the buttock, given the buttock retracted and saggy shape , like a half saw cut of the lower half of the buttock. In these circumstances , some times we can manage to expand intraioerstively the ata he’d skin and fill the whole buttock in a more proportional way and recover the whole spherical shape of the buttock. But frequently we can’t expand in proper way so we prepare our patients with multiple Pre op treatments to get the skin to expand and fill properly.
After we have the buttock ready for fat injection , then we proceed with the surgery. Excellent results are obtained in patients that were usually submitted to buttock lifting with long and visible scars without today cutting. It there are other problems Keith post massive weight loss patients related to the quality of fat that we can harvest. Usually , after major weight loss , patients lack fat in amounts that can help rebuild the buttock area properly and most importantly , there is a loss of quality fat after weight loss that few medical experts are aware off. Simply put, áreas that we usually harvest fat from in patients after weight loss , are mainly non fat areas and are actually made of sagging tissues and components of scaffolds that supports the skin made of bad collagen and structures empty of fats that we end up removing and re injecting into the buttocks of patients. Patients have to be aware that the small amounts of fat among those sagging tissues that we have harvested , has to be taken care and very carefully treated , so the fat that was injected don’t disappear. Because of that patients have to avoid sitting for at least 3-4 weeks , if they were treated to correct post massive weight loss due to the fat poor quality
Because of all this , we usually recommend surgery associated , with buttock Intra muscular implants plus fat to fix the buttocks in post massive weight loss patients. Fat in those cases are used to give contouring of the buttocks for a more round shape and the implants used to give proper projection of the buttock.
BREAST LIFTING AND AUGMENTATION FOR SAGGY BREASTS AND POST MASSIVE WEIGHT LOSS PATIENTS
Female Patients in the Middle East area are usually patients that have more than 3 children sometimes 5-6 pregnancies are the rule. And there are those young ladies that , never married before , were overweighted and then they were submitted to gastric sleeve or bypass and ends up developing very saggy breasts with no structural gland to rebuild them. So a lot of major variations that give us a more difficult task to achieve a similar and beautiful results to reshape again those breasts.
Patients have usually
1-hanging thin breasts with hanging nipples looking down to the floor
2- very thin skin
3-overstretched skin with lots of stretch marks
4-huge degree of deficient gland tissue without any firm support
Those are very difficult variables to deal with and sometimes need more than one procedure to fix everything properly
But all of them can be addressed almost perfectly at the first time
How do we do it?
Today , thanks to properly harvested fat in small lobules or small drops , the survival rate can be excellent but it is very time demanding surgery. So we combine breast lift surgery plus implant insertion u der the muscle to isolate the implant from the greats and decrease the rate of breast implant rejection that was very high before in this type of surgery. Besides we Inject fat around the implant in the muscular layer u der direct vision to avoid dangerous movements that can perforate the rib cage and we add under the skin as well. So a combined 3 surgeries in one help us achieve excellent results that were much more difficult to get before the fat revolution. But patients have to be alerted that the fat injection may not have a complete retention and more would be needed in another procedure , smaller procedure oriented to add o my fat to achieve a more soft touch for the breast.
Remember that patients with such breasts without any soft tissue before the surgery need lots of fat to get the old soft natural touch that only fatty tissue can provide for the patient. This is why we usually alert the patients in the first consultation that possibly one extra procedure would be necessary …not to shape the breast or to lift it but just to transform in a more natural feeling breasts without the implant feeling.
As usual patrol ends that undergo such surgery have to prepare for the surgery and every health problem has to be fixed prior to the surgery or at least under control and exams of the breasts such as mammogram and ultrasound are mandatory to compare to after the surgery if needed by the patient on her future breasts control.
Vaser Liposuction is surgery done to break up fat using ultrasound waves. The fat is then removed through traditional liposuction methods. This type of liposuction is becoming increasingly popular since the downtime is minimal and patients are able to make a quick recovery and carry on with their everyday lives.

Can it be done ?
The overall answer is yes. However, a proper scientific way to help those patients is to build a general guidelines to analyze those patients and their particular set of problems and the correct path to intervene and achieve the best outcomes.
What type of skin for every area to be approached?
Thickness , stretch marks , sagging tissues between muscles and skin are the best references to evaluate skin quality and its tolerance to surgical and devices aggression.
What is the degree of sagging tissues involved per area?
Patients with sagging tissues in the upper abdomem and back are easily fixed whereas sagging skin the lower abdomem under the umbilical scar are much more difficult to achieve a proper skin retraction.
For every procedure patients are going to get benefits but with some setbacks
We know today that sagging skin or rolls around the body back and abdomem can be successfully approached by agressive liposuction and when combined with tissue generated heat a better retraction fo the skin is observed.
But what degree of aggressiveness can be exerted in the liposuction that can be tolerated and more importantly can be added to the use of heat generated device under that thin deflated skin? A major hurdle is that agressive liposuction can lead to a high degree of skin retraction that solve the rolls that patient present mainly in the back and upper abdomem, but will be difficult to add Jplasma under a deflated and thin skin in the areas mainly waistline
Thicker skin like in the back and arms or thighs submitted to aggressive liposuction May withstand better heat generated devices to help retract suctioned skin.
Another approach would be to do in 2 times surgery, the first one to deflate completely sagging skin and rolls, and then on the second surgical time to apply VASER and Jplasma to the areas. But due to costs and work leave absence difficulties, a 2 times surgical procedure rarely entice patients
So patients have to be aware of possible complications, where the main are post liposuction burns and skin loss and discoloration.
Liposculpture is an improved form of the liposuction technique.
